Below are the intake forms that will need to be completed at the intitation of services. Many of our clients choose to print them off and complete them prior to their first appointment. While some people choose to mail them (if there is adequate time before their first appointment), we generally encourage people to physically bring the forms with them to the first appointment. Please Note: We will also need to obtain a front and back copy of your insurance card at the time of the first appointment.

 

-For adults (18 and older), please complete the Adult Intake Packet.

-For children (those under age 18), the Adolescent/Child Intake Packet will need to be completed by

a Legal Guardian.

 

The Release of Information forms only need to be completed if there is someone with whom you would like us to communicate. This form is generally completed with the name of family physicians, teachers, or other significant persons in the client's life. Please Note: Even after this permission is given, it can be revoked in writing at any time.
